In order to complete a codex entry, you'll need to scan the same target multiple times. If it shows in green in your scanner, it's complete, and you don't need to scan it anymore (it won't let you anyway). You can review these entries in your codex between the quests. If there's a laurel crown next to an entry, it means it's complete.
