If that old version supports utf-8 csv imports, you might be able to import the apkg into an Anki 2.0/2.1 (you can install it into another directory, though I don't know whether it will try to convert your profile, so maybe do that on another computer or at least another user), export to csv, make a nice template in your old version, and import the csv file with that note type.
